Attendance
-
In case of
absence or lateness, the parent/guardian must call the
absence line at 848-2610. A message can be left on the
absence line at any time but no later than 8:45 a.m.
If you forget to call the absence line, the office will call
to verify the student's absence.

Notification is required for
each day a child is absent.
-
In the event that a child is
absent for 5 or more consecutive days, a physician's note is
required for re-admission to school.
Health Services and
Screenings
-
Every child is screened
annually for height, weight, blood pressure, vision and
hearing. Calculation of the body mass index is done
for each student and reported via the grades 3 - 6 health
portfolios created in P.E. class. Scoliosis screening
is performed for all 5th grade students. Parents will
be informed of any deviations found during the screening
process.

The school attempts to
provide an environment in which children are safe from
accidents. If an accident or sudden illness occurs, first
aid will be administered and the child's parents will be
notified by the school nurse, if necessary. If an
injury or illness occurs at home, treatment should be given
at home. If treatment needs to be continued at school,
a doctor's note is required describing the type of
treatment.
Medications

Students requiring
medications at school need documentation from their doctor.
Forms are available from the Health Office. Physician
orders are required for over-the-counter medications as well
as prescriptions.

ARE NOT PERMITTED TO TRANSPORT MEDICATIONS!
Registration Requirements

Registration for kindergarten
students takes place in the spring prior to the year of
entrance. Each child must be five years of age on or
before October 1st. Each child participates in Dial 3,
a kindergarten readiness screening.

Parents/guardians must
provide the following for any student entering the district

Proof of Residency

Current Immunizations
and Physical Examination

Transfer card (if
transferring from another school)
General Information
-
Your child's medical information is
confidential. The nurse should be aware of
situations concerning your child's physical, mental
and emotional health, including daily medications
that your child takes. This is important
information should your child become injured or ill
at school.

If your child suffers from a fever,
vomiting, diarrhea, or any other contagious illness,
it is school policy that he/she remain out of school
for 24 hours after the symptoms have cleared.

Please contact the main office or
school nurse if your emergency contact numbers
should change. This is extremely important if
it is necessary to contact you for an injured or ill
student.
